No, a physical referral is not required to schedule the initial occupational therapy evaluation. If you plan to submit paperwork for your insurance company for any out-of-network benefits, a physical referral may be required.

No, Little + Well is a fee for service practice. We do not bill insurance. Upon request, you will be provided with a detailed receipt that you can submit to your insurance for any available out-of-network benefits. We accept cash, check, or credit (including HSA payment).

All evaluations and therapy appointments will take place at the Little + Well clinic located at 408 South Main Street in Fort Loramie.
